Published purpose
The project will leverage technology automation to improve communication and coordination through the continuum of care. By digitizing paper processes and creating capacity building and efficiencies, to capture and report on data collection, and metrics, for this patient population. The focus is on reducing ALC days and improving the continuation of care from clinical practice to the home.
