Dedicated to vulnerable populations at high amputation risk, the firm will onboard 4 Long Term Care facilities, 2 community health centers, and 2 rural regions. The firm's aim: an integrated holistic service. The firm will iterate, develop, and create a distributed provider network sharing valuable data to mitigate complexities. Collaborating with local infrastructure, they showcase value-based care as a viable method in Ontario and beyond. This 3 month project will focus on the North Toronto Region, Muskoka Region and Grey Bruce.
